At Associated Speech and Language Services we realize how valuable your time is. We have made our patient background and other forms available for on-line entry, or as a download for those forms requiring a signature. Feel free to use them at your leisure.

Please complete all applicable forms prior to the initial meeting with one of our clinicians. This information will assist in preparing for you or your child’s evaluation and subsequent services, as necessary.

Of course you’re always welcome to fill out all our forms the old fashioned way, scribbling on a clipboard in our comfortable waiting area.
